Authorization Agreement for Direct Debit Payments (ACH Debit)
Pay your bill automatically each month from your checking account. There is no additional fee for ACH payments set up through the Authority office, and your payment is processed on the due date each month.
To start, stop or change ACH payments:
- Download and print the ACH Authorization form below.
- Check Start ACH, Stop ACH or Change Account Info, then complete and sign the form.
- Return the form with a voided check or a letter from your bank showing your routing and account numbers. Mail it to the Municipal Authority of the Borough of Midland, 946 Railroad Avenue, Midland, PA 15059, place it in the office drop box located next to the entrance, or bring it to the office (Monday, Tuesday, Thursday and Friday, 8:00 a.m.–4:00 p.m.; closed Wednesday and 12:00–1:00 for lunch).
For your security, bank information is not collected through this website. ACH set-up is complete once the Authority has received your signed form and voided check. The customer will be billed the current bank rate for insufficient funds.
