Application for Water and/or Sewage Service
Property owners — 5/8″ metered service
The undersigned applicant, owner of the premises listed below in Beaver County, Pennsylvania, requests to be supplied with water and/or sewage services by the Municipal Authority of the Borough of Midland (the “Authority”) for the purpose shown in this application and for no other purpose. The applicant agrees to pay for such service within 15 days after receipt of billing for metered delivery at the regular scheduled rates, and to comply with the Authority’s rules and regulations.
The applicant agrees to claim no damage on account of stoppage of the flow of water resulting from an accident, or when a stoppage is necessary to make alterations, repairs or improvements, and no damage from a ruptured water or sewage line resulting from an accident not due to the negligence of the Authority. The applicant shall keep all plumbing fixtures on the premises in good repair and promptly stop all leaks.
The applicant agrees to pay for water and sewage services until written notice to discontinue service or notice of the sale of the premises is given to the Authority. The minimum rate for a 5/8-inch meter is $57.06 per month for the first 2,500 gallons.
